Please use the following guide to help you fill out your own intake form! This intake form will provide the basis for your branding, so take your time on it. If you find yourself lacking an answer to any of these questions, search Google or Pinterest for some inspiration and to get clarity. If there’s a color palette that you love, try asking yourself what you love about it and use that answer to inform the next color palette you like. If there’s something that you really don’t like, again, ask yourself why you feel so repulsed and use that information as guidance. After you submit this intake form, we’ll chat so that I can make sure I understand what you’re going for before I start on any mock ups.
Name
What’s your name?! This is not your business name, it’s your personal name.
What is the best email to contact you on? Keep in mind, this is the email that you will receive your invoice on, as well as invitations to our video chats, access to your finished branding, and any other communication.
Business or Brand Name
For this guide, I am going to use the fictitious dog obedience business, Luna Commands (inspired by my own dog, Luna)
Tell me about your business or brand
Luna Commands is a dog obedience business that focuses on training dogs in a kind, yet stern way. Through our methods and processes, we build trust with each dog, on an individual level and tailor the training program to fit their own way of learning and the rewards they most prefer. We have a board-and-train program as well as an owner and dog program, in which the owner attends training lessons with their dog. We teach dogs to be obedient to all of the standard commands, as well as how to heel and bark when prompted. We strive to work with the dogs, and not break their spirits or playful natures.
As succinctly as possible, what is your message?
You can be kind and strict. Your dog can be spirited and obedient.
3-5 adjectives that describe your brand
Modern, sleek, graphic, with elements of fun.
*If you need help finding adjectives, you can use a thesaurus or google “list of adjectives”
Color Palette
You can easily find color palettes by searching on Pinterest or Google “color palettes”. Narrow down the search results by using an adjective. I made the following three color palettes (I create a new one every week on my blog!), however, I could have searched “Neutral color palettes” or “earthy color palettes” or even “nature color palettes” to get similar results. Pinterest is a particularly great place for search results of this type. Pull at least three color palettes, for variety. You can also submit photos here that have colors that match the vibe of your brand.
Imagery
Sometimes images are better at portraying the feel you’re going for more than words do. Although two of these photos depict dogs, your images don’t need to have anything to do with your business or industry. The important thing here is that the photos convey the feeling and the emotion that you want your brand to communicate. For example, I chose the first photo because I love the serene feeling of it. I love knowing I can take my dog on a walk and have it be a peaceful experience in which we can both enjoy nature. The second photo conveys the feeling of being obeyed by my dog, of her waiting for her next directive, her discipline, and attention. The third photo brings in the feelings of joy of being with my dog and enjoying each other’s company.

The branding that you like says a lot about what you want in your branding. While I again chose “dog themed” brands, you can choose branding from any industry and business
